Pre-test ini disusun untuk mengukur pemahaman awal peserta terkait konsep dan penerapan Root Cause Analysis (RCA) sebelum mengikuti pelatihan. Pertanyaan yang diberikan mencakup prinsip dasar RCA, teknik investigasi, identifikasi akar penyebab, metode analisis, hingga penerapan corrective and preventive actions (CAPA). Hasil pre-test akan digunakan sebagai acuan untuk mengetahui tingkat pemahaman peserta sebelum materi pelatihan diberikan.

Post Test Root Cause Analysis
This post-test is intended to evaluate participants’ understanding after completing the Root Cause Analysis (RCA) training program. The questions are designed to measure comprehension of RCA concepts, root cause analysis techniques, incident investigation processes, RCA tools, and the implementation of corrective and preventive actions. The post-test results are expected to demonstrate the improvement of participants’ knowledge and competency following the training.

Root Cause Analysis

Module 1 – Introduction to Root Cause Analysis
This module introduces the definition, objectives, and importance of Root Cause Analysis (RCA) in improving safety, quality, and productivity. Participants will learn the difference between treating symptoms and eliminating root causes to prevent recurring problems.

Module 2 – Types of Causes and Problem Definition
Participants will learn to distinguish between symptoms, immediate causes, contributing causes, and root causes. The module also discusses common human, technical, and organizational factors that contribute to incidents, as well as how to develop a clear and accurate problem statement.

Module 3 – RCA Tools: 5 Whys and Fishbone
This module explains the principles and application of the 5 Whys technique and Fishbone (Ishikawa) Diagram. Participants will practice basic cause-and-effect mapping and learn practical tips to avoid bias during brainstorming and analysis sessions.

Module 4 – Investigation and Data Collection
This session focuses on gathering facts and evidence during investigations. Participants will learn simple interviewing techniques, methods for reviewing records, photos, and operational data, as well as approaches to validating information before conducting analysis.

Module 5 – Developing Actions and Reporting
Participants will learn how to translate identified root causes into effective corrective and preventive actions. The module also covers assigning responsibilities and timelines, preparing a simple RCA report structure, and communicating investigation findings to management.

Module 6 – Case Study and Group Exercise
In this practical session, participants will work in groups to conduct RCA on a sample incident or operational problem. They will apply the 5 Whys and Fishbone methods, present their findings, and receive feedback to strengthen their analytical and problem-solving skills.
